Important parts in italics. My comments in bold.

Toxic Faith (a Mamba Naga Priest) has been spotted in Menace Mausoleum lvl 1.
Discovered an evil altar cursing the area.
Jeane needs to be rescued.

I hunted all over and never found this monster. Seriously, I patrolled this level over and over and over. Never found it. This could have been my fault, but I don't think it was. I've had this happen a few times with artifacts, too.

Something really cool just happened there. I killed Toxic Faith, and apparently he got resurrected as a zombie by a named lich. While that was really awesome, I think the text explaining it could have been better. Because at first it looked like a glitch- you've killed toxic faith, a new group is taking advantage of his death, and it includes toxic faith. When a monster resurrects a dead named monster (which is awesome, let me add that again), it seems like there should be some customized text explaining what happened.

This one was less cool. I've got no idea what happened. I killed Grimbone, and then he immediately came back in a partnership with a named lurking death. Then he died. I don't think I killed him, though this character does toss around earthquake spells pretty freely and might have gotten him through a wall. But I'm pretty certain his partner did Grimbone in. I don't know why.

War quests cannot be completed unless you officially have the quest. If you don't officially have the quest, you can kill as many Eternal Wizards as you want and never win. This may not be a bug, I'm just noting it.

Overall I had a really good time. I'm using a trickster/druid that focuses on earthquakes, so I die a lot from my own attacks, but I dish out so much damage so fast that I mostly make up for it. I'm not 100% on whether Stealth is modifying Earthquake- Stealth is supposed to improve physical damage, which Earthquake is, but I'm not sure if it actually does. Its very hard to tell since so many things modify Earthquake damage right now.

My general thought has been that the +Phys from Stealth would only come into play with Melee skills with a physical component. Though, I could see how it would make sense on something like an Arrow Shot or Shuriken.

I'm pretty certain it works on arrows. I did a little testing way back in the first edition of the beta, and I think I remember it working then.

I assume it works on Shuriken. It would be awfully sad for it not. Shuriken is secretly a weapon attack, after all. It uses your weapon damage now, plus a bonus exactly like a regular weapon skill, except with a ranged effect.

I'm getting a feeling that it doesn't work on Earthquake though, since its not really a weapon attack. That's kind of a shame, because I really like this character otherwise. I think I may stick with him even though Stealth isn't doing much for me except helping me maneuver. If it doesn't work on Earthquake, it should probably be reworded to apply to melee attacks, arrows, and shuriken instead of "physical damage."

I've had this same problem a few times myself. In my current town, my rogue is supposed to kill a particular boss on level 5, and has scoured every bit of level 5 multiple times, and clicked on every single wall, and the boss monster simply is not on level 5. She had the same exact problem on level 4 with another boss, but that one was an "avenge this dead person" optional quest, so she simply ended up declining it. The level 5 one is required for winning though.

I think the blank spot that killed your character was an Earthquake... for some reason, the game seems to count the "epicenter" of earthquakes as monsters, albeit monsters with no name.
I've seen earthquakes spawn as champions and even elites, you can tell by the green or yellow aura popping up somewhere near the center of that "dust clouds" effect that telegraphs them.
You can even select and attack them if you're fast enough, since they disappear on their own a few seconds later.

Damn those earthquakes and cave-ins demanding their right to promotions just like other monsters. :)
